What types of information do we collect about you?
When you apply to Four Point HR and/or its affiliates and partners for insurance, you disclose information about yourself. In addition to the information we receive from you on applications or other forms, we may collect nonpublic personal information about you from the following sources:
Information about your transactions with us, our affiliates or others; and
Information we receive from consumer reporting agencies or other third parties. If we were to order any kind of consumer report, upon request we will tell you how to get a copy of that report. Information about your transactions within our company, our affiliates, the insurance companies we represent or others with whom we may contract.
How do we protect the confidentiality of information about you?
We restrict access to information about you to those employees or third parties who need to know that information to provide products or services to you, and for other reasons permitted by law. We maintain physical, electronic and procedural safeguards to guard your nonpublic personal information.
With whom do we share the information?
We do not disclose any nonpublic personal information about our customers to anyone, except as permitted by law. In some cases, this may mean information can be disclosed to third parties without your authorization.
Four Point HR follows these privacy practices even when a customer relationship no longer exists.
You have the right to obtain access to certain types of information we have collected about you, and you have the further right to request correction of the information if you feel it is inaccurate. We would also be pleased to tell you about our policies and procedures for the privacy of your information. Such requests should be made in writing.
